Protocol Description
This study has two parts. Part 1 uses an experimental combination of drugs and Part 2 uses a new combination of standard drugs:
Part 1: Window Therapy with irinotecan and vincristine
We want to see if an experimental pair of drugs, irinotecan and vincristine, are effective in treating tumors. These drugs have been found to shrink some Wilms tumors in laboratory studies with animals. These drugs have also been used to treat children with rhabdomyosarcoma, and were shown to shrink these tumors in most patients. Using this combination of drugs for subjects with anaplastic Wilms tumor or rhabdoid tumor is experimental therapy. This is called Window Therapy because the experimental combination is given before any of the standard drugs used to treat these tumors.
Part 2: Regimen UH-1 or UH-2
We want to see if a new combination of five standard drugs (called Regimen UH-1) lessens the chance of the tumor returning compared to the currently used treatment regimens that use three or four drugs. Also, the doses in Regimen UH-1 are higher compared to the doses commonly used. The drugs used in Regimen UH-1 are vincristine, cyclophosphamide, doxorubicin, carboplatin and etoposide.
Eligibility Criteria
Children and adolescents with anaplastic Wilms tumor, malignant rhabdoid tumor, clear cell sarcoma of the kidney, and renal cell carcinoma
