Forms for Compliance Requirements and Special Approvals
Protocols involving the participation of human subjects or identifiable private information must be reviewed and approved by the Institutional Review Board (IRB). Forms relative to this process may be accessed from the following website:
Protocols involving animals must be approved by the Institutional Animal Care and Use Committee.
- Application to Use Animals in Research or Teaching
- Annual Update of Approved Protocol
Protocols involving recombinant DNA, infectious and biological agents and/or hazardous chemicals must be reviewed by the Institutional Biosafety Committee.
