OGC - Procedures for Submitting Applications
The Office of Grants and Contracts must review all grants and contracts before they are submitted to a sponsor for funding consideration.
Grant and contract applications must be received at least 5 days before the submission deadline so that applications may be reviewed appropriately and institutional signatures obtained in a timely manner.
The following forms must accompany each grant application. (Follow the link to a form and save it to your hard disk.)
Conflict of Interest Questionnaires - for key personnel; must be submitted with each grant application.
Grant Application Routing Form - must be signed by the appropriate Department Chair (or designee) and be submitted with each grant application.
Clinical trial agreements should be forwarded to the Grants and Contracts office as soon as possible so that negotiations with Sponsors and/or Clinical Research Organizations may proceed while protocols undergo review by the Institutional Review Board.
