To add new personnel to an experimental protocol, modify responsibilities of approved personnel or remove personnel from an approved study, complete an Administrative Change Request Form. Submit the completed form with original PI signature as a PDF to email@example.com. You must be approved as personnel on an IACUC protocol before requesting training and facility access.
Pre-requisites must be completed before requesting to be added to an experimental study:
- CITI Training
- Enrollment in Occupational Health and Safety Program
- Conflict of Interest
- Submit the following forms to the IACUC office at firstname.lastname@example.org.
- Demographic Form for Controlled Substances (submit as PDF with original signature)
- GR050 Authorized User Screening Form (submit as PDF with original signature)
- CV (submit word .docx or PDF)
CITI Regulatory Training
Northwell Health uses the Collaborative Institutional Training Initiative (CITI) web based training program. If you have not utilized CITI before, create a new account by visiting the citiprogram.org website to register. Your participating institution is “Northwell Health“. If you will be working with live animals, please designate yourself as a Laboratory Animal Researcher and you will be automatically enrolled in all courses in the curriculum.
Enrollment In Occupational Health & Safety Program (OHSP)
All research personnel participating in animal-based research must enroll in the Occupational Health and Safety Program and obtain clearance from Employee Health Services (EHS).
For Northwell Health Personnel (including Hofstra Medical School Students): Complete the Individuals Engaged in the Care and Use of Research Animals initial questionnaire form. Send completed questionnaire via email to email@example.com or fax (718) 470-1232. Clearance is valid for a year. Personnel must submit an annual questionnaire form Individuals Engaged in the Care and Use of Research Animals – annual questionnaire.
For Students (non-Northwell Health employees/non-Hofstra Medical School): Complete the Students Engaged in the Care and Use of Research Animals form. Obtain clearance from your primary care physician to engage in animal based research activities. Submit the physician attestation page only to firstname.lastname@example.org.
Conflict Of Interest (COI) Disclosure
Individuals responsible for the design, conduct or reporting of research are required to complete an External Interest Disclosure (COI Disclosure) electronically through COI Smart at least annually and should be updated within 30 days of any new significant financial interest.
